This form allows one player that lists an 18+ team as their "Primary Team" to register & provide contact information to the Mid-Iowa Baseball League for the 2026 season. Once completed, the player is officially waivered and rostered.

STATEMENT OF PLAYERS AGE & FITNESS
I hereby warrant to the Mens Senior Baseball League (MSBL), Inc. / Mens Adult Baseball League (MABL), Inc & to Mid-Iowa Mens Senior Baseball League (MIBL) that I am at least 18 years old & that I have otherwise been declared qualified by the Board of Directors of the MIBL. I further warrant that I am in good physical shape & do not suffer from any illness, disease, or ailment preventing me from engaging in the activities of the MIBL. I understand that I can only participate in the MIBL if I meet the age requirement warranted hereto unless I have been specifically exempted from said requirements by the MIBL. I will provide, if questioned, a true & correct copy of my current drivers license or other valid proof of age. I understand that I may be barred from all further participation in the MIBL & acknowledge that my team may be penalized by the MIBL if any information furnished has been falsified for purposes of gaining admission to the MIBL.

PARTICIPATION
(1) I shall participate in the MIBL effective in the calendar year listed above as a roster member of the MIBL team(s) listed above. (If playing for multiple teams, I have listed all that are applicable.)
(2) As a member of the MIBL, I may also be eligible to participate in the MSBL/MABL World Series, Fall Classic or other tournaments & games organized & recognized by the MSBL/MABL/MIBL sponsoring same & agree to the rules governing same.

AGREEMENT TO BE BOUND
In consideration of being allowed to participate in any way for the MSBL/MABL/MIBL, its related events & activities, I the undersigned, acknowledge, appreciate & agree that:
(1) I will not possess or consume any alcoholic beverages or illegal drugs at any time on the playing field, in the dugouts, or any other areas near or around the playing field, including the parking lots of any high school, college, or municipality used by the MIBL (the Facilities).
(2) I understand that smoking or the use of other tobacco or vaping products is prohibited at the Facilities.
(3) I understand that fighting, threats, &/or abusive or profane language will not be tolerated & are grounds for suspension &/or expulsion.
(4) I understand that the possession of firearms or weapons are strictly prohibited at the Facilities.
(5) I shall abide by all league rules & I understand that a violation of any rule or regulation set forth herein or referred to may result in a discharge of myself or of my team including forfeiture of games, suspension from play, or permanent disbarment from the MIBL. I understand that certain acts on my part may result in prosecution & or other legal sanctions, including reimbursement for damages incurred.
(6) I understand that the primary method of communication to players & managers will be via the MIBLs website (www.iowabaseball.org). This includes, but is not limited to, game scheduling, notification of rain-outs, umpire scheduling, board meeting minutes, manager meeting minutes, protests, grievances or any other pertinent communications.
(7) Participation includes possible exposure to & illness from infectious diseases including but not limited to MRSA, influenza, & COVID-19. While particular rules & personal discipline may reduce this risk, the risk of serious illness & death does exist.

RELEASE & WAIVER OF LIABILITY
In consideration of being allowed to participate in any way for the MSBL/MABL/MIBL its related events & activities, I, the undersigned, acknowledge, appreciate & agree that the risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis & death, & while particular skills, equipment & personal discipline may reduce this risk, the risk of serious injury does exist; & I KNOWINGLY & FREELY ASSUME ALL SUCH RISKS, both known & unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, & assume full responsibility for my participation; & I willingly agree to comply with the stated & customary terms & conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation & bring such to the attention of the Company immediately.

INDEMNIFICATION & HOLD HARMLESS
I, for myself & on behalf of my heirs, assigns, personal representatives & next of kin, HEREBY RELEASE, INDEMNIFY, & HOLD HARMLESS MSBL/MABL/MIBL, their officers, officials, agents &/or employees, other participants, sponsoring agencies, sponsors, advertisers, &, if applicable, owners & lessors of premises used for the activity (Releasees), WITH RESPECT TO ANY & ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property associated with my presence or participation, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

THE FOLLOWING ITEMS COVER OUR NATIONAL RELEASE & PLAYER PARTICIPATION AGREEMENT:
I hereby agree & consent to the following parameters as conditions of participation in the MSBL/MABL/MIBL:
(1) I will observe all rules as established by the MSBL/MABL/MIBL.
(2) I understand that fighting, physical abuse of players, umpires, or spectators, & the use of abusive or offensive language will not be tolerated by the MSBL/MABL/MIBL & violation of this rule could result in my banishment or suspension from the league & forfeiture of all fees paid.
(3) I realize that MSBL/MABL/MIBL & facilities do NOT possess a defibrillator.
(4) I realize that there is no guaranteed playing time on any given team, associated with regular season & tournament play.
(5) I certify that I am, or will, turn 18 years of age before taking the field or am or will turn 28, 40, 48 or 58 years of age during this calendar year.
I realize that the total responsibility for any injury, accident, incident, illness, or death to me or my person while participating in ANY MSBL/MABL/MIBL activity, game, practice or function, including, but not limited to any MSBL/MABL mandated or scheduled functions are solely mine. I fully realize that any costs incurred for any reason are mine.
By signing this agreement I release the MSBL/MABL/MIBL from any liabilities or cost. I fully agree that the terms & conditions of this agreement are binding. I have read this release of liability & assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights by signing it, & sign it freely & voluntarily without any inducement.
